Weitgehend unbekannt in der Bevölkerung und auch in Ärztekreisen ist das mögliche Auftreten von Konvergenz-Schielen bei Senioren aufgrund des Alterns und ohne fassbare neurologische Ursachen. Die Patienten leiden unter Doppeltsehen, das, als schicksalhaft aufgefasst, oft über viele Jahre unbehandelt bleibt, jedoch mit Brille oder Operation gut behandelbar ist. Zusätzlich leiden diese Patienten oft an geriatrischen Augenerkrankungen wie z.B. Katarakt, Glaukom, Makulopathie, die ihrerseits zur Entstehung von Binokularstörungen beitragen können. Diese äußern sich oft nicht als Diplopie, sondern z.B. als Bildunschärfe,  “Sehen von Wolken” oder durch wiederholtes Zukneifen eines Auges. Solche Angaben von Patienten werden als “maskierte Doppelbilder” aufgefasst. Das Buch enthält Hinweise zur Behebung dieser Störungen und dadurch zur Verbesserung der Lebensqualität der Senioren.

Der Band enthält Studien über die Ursachen und zum besseren Verständnis nicht korrigierbarer Doppelbilder, des sogenannten “Horror fusionis” und anderer ausgewählter neurologischer Erkrankungen.

Das Buch ist kein Lehrbuch für Orthoptik, vielmehr eine Ergänzung zu  umfassenderen Werken. Der Inhalt basiert auf theoretischen Studien wie auch auf persönlicher praktischer Erfahrung, welche die Autorin in 50-jähriger Tätigkeit sammeln konnte sowohl als Ophthalmologin wie auch als Strabologin an der Medizinischen Universität Wien und in der eigenen Praxis

Elderly patients are often afflicted with the onset of a convergent strabismus as a sign of aging, without any other neurologic disorder. Unfortunately, physicians and even ophthalmologists are generally unaware of this fact. Consequently, such patients fail to find timely help for their double vision. In addition, there are other geriatric alterations (such as cataracts, glaucoma and age-related maculopathy), which are connected with binocularity disorders but do not always result in double vision. These masked diplopia are only perceptible as the closing of one eye and the patient’s complaints of seeing clouds. Erroneous diagnoses have a dramatic outcome for many elderly people whose ability to read is vitally important for their quality of life. Studies on this topic and suggestions for improving the patient’s situation are contained in this book. The book also covers intractable diplopia, or “horror fusionis”, which is particularly difficult to understand. The book is not a textbook of orthoptics. It seeks to provide advanced training for ophthalmologists based on the author’s personal experiences collected over fifty years of practice as an ophthalmologist and specialist for strabismology at the Medical University of Vienna’s eye clinic and in her own office.
